"Effective 12:01am Sunday October 31 Middlesex London Health Unit has implemented mandatory vaccination checks will be required for anyone 12 years of age and up who enters an indoor area of a sports or recreational fitness facility to participate in, coach, officiate, or be a spectator for such sports activity.”
Players, coaches and officials that have been exempt up from proof of vaccination will be required to show proof of vaccination and identification or be denied entry into the facility.
Any player, coach, official, spectator who is 12 years of age or older must show at the security check point:
Proof of full vaccination (2nd dose + 14 days) (Provincial Issued Vaccine QR code passport or previous paper copy
Valid Identification showing name and date of birth (can be photocopy)
Completion of daily covid prescreening questions
What is acceptable for vaccination verification:
New provincial enhanced vaccine passport QR code:
Scan the QR code using the provided phone and Verify Ontario app
Previous paper copy of vaccine proof is acceptable
(Not accepted is the non-provincial QR code app. that people have downloaded)
What is acceptable for valid identification:
12-17 years old can use documentation that shows name and date of birth (no picture required)
18 + may have photo identification that can be used but not a requirement as name and date of birth is all that is required
Photocopies are acceptable
Must provide written and signed Dr’s note + reason and effective time period.
No other exemptions such as religion will be accepted for entry.
Each person must provide, at the point of entry, proof of vaccination and proof of Identification for each time entering facility
