OMHA/OWHA - Team Preseason Signoff Sheet - Rowan's Law

Team-Rostered Trainers must attest to have seen a signed acknowledgement form for each player on their team. Team preseason sign off sheets can be downloaded by trainer in word. Player names can be filled in prior to seeking signature of at least one parent for each player on the team. Once signatures for all players have been obtained arrangements can be made to drop off a hard copy or the trainer may choose to scan and send an electronic copy to the Head Trainer of the association for inclusion in our files. A copy of the original signed acknowledgement form will be retained by the parents of each player.
